Description

ACE-031 (also called ramatercept) is a recombinant fusion protein that joins the extracellular domain of the human activin receptor type IIB (ActRIIB) to the Fc region of human IgG1. It acts as a soluble “ligand trap” that binds myostatin and related TGF-β family ligands before they reach their cell-surface receptors.

What is ACE-031?

Myostatin (GDF-8) limits skeletal muscle growth by signalling through ActRIIB. Because ACE-031 carries the receptor binding domain itself, it also sequesters other ActRIIB ligands such as activin A and GDF-11, so its effects are broader than a myostatin-specific antibody. In animal studies, ActRIIB-Fc proteins produced rapid increases in muscle mass.

ACE-031 entered clinical development in Duchenne muscular dystrophy, but the programme was stopped after participants developed nosebleeds and small dilated blood vessels (telangiectasias), which have been attributed to off-target binding of BMP9 and BMP10. That history makes it a reference point in research on how ligand selectivity shapes the safety of activin-pathway inhibitors.
