Tesamorelin is a full-length 44-amino-acid GHRH analog with an N-terminal hexenoyl group, studied for DPP-4 resistance and growth hormone axis research.
Tesamorelin is a synthetic analog of human growth hormone-releasing hormone (GHRH) that contains all 44 amino acids of the native peptide. A trans-3-hexenoyl group is attached to the N-terminal tyrosine. That modification sets it apart from shorter GHRH fragments such as sermorelin.
The N-terminal region of GHRH is where dipeptidyl peptidase-4 (DPP-4) cleaves the hormone, rapidly inactivating it in plasma. Adding the hexenoyl group protects this site, giving tesamorelin greater stability than unmodified GHRH while keeping its activity at the pituitary GHRH receptor. Receptor binding raises cAMP in somatotroph cells and increases synthesis and pulsatile release of growth hormone, which in turn stimulates hepatic IGF-1 production.
Clinical research with tesamorelin has used visceral adipose tissue, liver fat, and lipid profiles as study endpoints. Because the compound stimulates endogenous GH rather than replacing it, it suits experiments that require somatostatin feedback and the natural secretory rhythm to remain intact. It is often compared with sermorelin and CJC-1295 to examine how different stabilization strategies, N-terminal acylation versus amino acid substitution, affect GHRH receptor activation and resistance to enzymatic cleavage.
